Care Coordination

When someone reaches out for help, our care coordinators are ready to listen. We serve as a trusted starting point for anyone seeking mental health or recovery support. Our team works one-on-one with individuals and families to understand their needs, connect them to providers, and help navigate the path to treatment and recovery.
We follow up, provide encouragement, and ensure no one falls through the cracks. This compassionate, hands-on approach helps reduce barriers, foster trust, and make the journey toward wellness less overwhelming—and more hopeful.

Sometimes, the most powerful healing happens in community. Our peer and professionally facilitated support groups bring neighbors together around shared experiences—from grief and caregiving to ADHD, recovery, and parenting.
In these circles, participants find solidarity, understanding, and belonging. Every conversation helps reduce isolation, normalize mental health challenges, and create space for growth. Whether you’re taking your first step toward healing or continuing your recovery journey, you’ll find a safe, judgment-free place to connect.

Mental wellness is a community effort—and we’re proud to lead it. Through local events, trainings, and awareness campaigns, The Hub helps Philipstown residents learn, connect, and take action.
Our Narcan trainings, speaker series, and overdose awareness events give people the tools to respond in crisis and build empathy across generations. Events like Movement for Mental Health and the You Matter Awards make mental health visible, celebrate recovery, and inspire ongoing community dialogue.

For participants who are looking for ongoing support, our You Matter program offers regular wellness check-ins through personal outreach calls.

It’s one more way The Hub ensures that every person in our community knows they’re not alone—and that someone will always reach out to remind them they matter.

If you have any unused or expired medications in your home, including pet medications, you can safely dispose of them at DrugWorld of Cold Spring, 55 Chestnut Street, Cold Spring, New York. Open 9:00am-7:00pm Monday-Friday, 9:00am-6:00pm Saturday, and 9:00am-2:00pm Sunday.
A safe vape disposal box is located inside the Philipstown Town Hall at 238 Main Street, Cold Spring, New York. Open 8:30am-4:00pm Monday-Friday.
